If you are having engraving/face milling/sound slot work done, then your putter needs to be here by 7-29-08. Future refinishing batches will be started on the 15th of each month. Putters must be here 3 days prior to the 15th for refinishing and 2 weeks prior to the 15th for putters that need engraving/face milling/sound slots and then refinished.
A hard copy of this form should also be sent with the putter.
